This allows for an external extension to more easily and correctly add GrpcClientBuildItem / ClientInfo.

Will this work? I don't think the order is deterministic so anything that relies on ordering (such as overriding) won't be reliable.

The ordering is not an issue, it's the duplicates -- which could occur with external extensions,
but with this change they would be "squashed" within that HashSet per clientName.
